You are able to book appointments, order repeat prescriptions, cancel appointments, change your contact details and view your Summary Care Record online.

To access this service you need to attend the surgery and ask for a registration form.

We also need you to provide us with one form of identification as follows:

• A passport or driving licence, or other official photo ID or a bank card

plus

• A recent document with your name and full postal address on for example a utility/council tax bill or bank statement. (We do not accept Tenancy Agreements.)

This is to protect patient confidentiality, and to ensure that we are only giving access to the persons the records relate to. You must present original documents yourself in person.

Then when you return, a minimum of 5 working days later, (again bringing your ID) we will provide you with a printed sheet with instructions on how to access the system, and your unique username and password.
